INFORMATION TEXT WITHIN THE SCOPE OF THE LAW ON THE PROTECTION OF PERSONAL DATA

Specialist Dr. Şeyma DEMİRCİ attaches great importance to the protection of the confidentiality of all individuals’ personal information and all related processes.As Specialist Dr. Şeyma DEMİRCİ, in accordance with Law No. 6698 on the Protection of Personal Data (“KVKK”), and in the capacity of Data Controller, we would like to inform you about the following matters within the scope of our obligation to enlighten under KVKK, taking into account the data security of our patients and their relatives, and the importance we place on the privacy of private life and the protection of fundamental rights and freedoms.

1-) Which of Your Personal Data Do We Process?

In order to provide health services to our patients/clients, we collect various personal data in accordance with the principles and conditions set forth in the KVKK and other relevant legislation. The following data are collected by our clinic from our patients, from other healthcare institutions and organizations, or from patient relatives, for the purpose of carrying out medical diagnosis, consultation, examination, treatment, and care services.

  • Your name, surname, Turkish ID information, passport number, place and date of birth,
  • Your gender and marital status,
  • Your address, phone number, and email address
  • Your patient protocol number and other identity data by which you can be identified,
  • The data specified in the consent form you may provide in case of a medical intervention,
  • Your private health insurance or Social Security Institution data for the purpose of financing and planning healthcare services,
  • Your biometric data within the scope of services financed by the Social Security Institution,
  • All personal health data obtained during the execution of medical diagnosis, examination, treatment, and care services, including general and special category personal data such as your laboratory results, test results, examination data, check-up information, and prescription details,

It will be processed, recorded, stored, preserved, classified and transferred to authorized public institutions and organizations, laboratories that we cooperate with for medical diagnosis, web & software digital marketing agencies and CRM & software companies, private insurance companies, law offices, financial advisors, in order to provide information to prosecutors, courts and relevant public officials in matters related to public security and legal disputes that may arise, upon request and in accordance with the legislation; when necessary, for the purposes mentioned above.

3-) What are the Methods of Collection of Your Personal Data and the Legal Grounds for the Collection?

Your above-mentioned personal data are subject to the provisions of Articles 5 and 6 of Law No. 6698;

  • It is clearly stipulated in the law,
  • Provided that it is directly related to the conclusion or performance of a contract, it is necessary to process personal data of the parties to the contract,
  • It is mandatory for the protection of the life or physical integrity of the person who is unable to disclose his/her consent due to actual impossibility or whose consent is not legally valid,
  • It is mandatory for the data controller to fulfill its legal obligation,
  • It is mandatory for the legitimate interest of the data controller, provided that it does not harm the fundamental rights and freedoms of the data subject,
  • Protection of public health, preventive medicine, medical diagnosis, treatment and care services, planning and management of health services and financing

within the scope of the purposes specified in this Clarification Text;

  • With specific, explicit and legitimate purposes,
  • In connection with, limited and proportionate to the purpose for which they are processed,
  • It will be processed, recorded, stored, maintained, classified and transferred by automatic means (for example, camera recordings) or non-automatic means (for example, forms filled out by you) provided that they are part of any data recording system (for example, forms filled out by you) in order to be kept for the period stipulated in the relevant legislation or required for the purpose for which they are processed.

4-) What Are Your Rights Regarding the Processing of Personal Data and How Can You Access These Rights?

Article 20 of the Constitution stipulates that everyone has the right to be informed about personal data concerning him/her. Article 11 of the LPPD lists the rights of the personal data subject under the law.

You can submit your requests regarding your rights under the KVKK to Article 5 of the “Communiqué on the Procedures and Principles of Application to the Data Controller” to Uzm. Dr. Şeyma DEMİRCİ in writing, secure electronic signature, mobile signature or by using the e-mail address previously notified to the data controller by the data subject and registered in the system of the data controller.
